Here are some things that are required for your travel claim.
• Appointment confirmation.
We require an appointment letter for an up-coming appointment from the hospital, clinic, or doctor that is requesting to see you, or a doctor's note stating you attended a pervious
appointment (reimbursement available).
We also require our clients to return confirmation slips (that are put into envelope with the travel) to the travel office.
Submitting confirmation slips/doctor's notes
Appointment confirmations/doctor's notes can be dropped off, faxed, texted or emailed to any of the medical transportation office workers. (we do not accept hospital bracelets as confirmation)

"Cheque run" times.
With the large volume of clients we accommodate daily. Peguis med trans does 2 "batches" of cheques daily, 1 in the morning and 1 in the afternoon.
Morning cheque run is approximately at 10:00am
Afternoon cheque run is approximately at 2:00pm

Cheque pickup
Travel cheques and reimbursements can be picked up from 9:00 a.m. -12:00 p.m. & 1:00 p.m. - 5:00 p.m. daily.
Notice: We ask our clients to give us at least 2-3 days notice of appointments so your travel cheque will be done on time.
We also ask our clients to give us 1-2 weeks notice if needing to be taken to an appointment by Med Van. Our vans book up very quickly, so make sure to notify us as soon as you are notified of an appointment to check availability.
"No shows "and missed appointments.
Any appointment that a client claimed travel for and did not attend, the client will "owe us a travel"; meaning the next appointment that is scheduled for that client will either have to utilize our Med Vans with no meal monies or go to the appointment out of pocket.
